N.Y. man sheds more than half his weight

N.Y. man sheds more than half his weight - After man loses nearly 200 pounds, he collects on bet. When Fulton, New York resident Jay Wornick set out to lose some weight at the urging of his wife, he didn't have high hopes. Weighing in at more than 360 pounds this past January, Wornick thought, at best, he'd lose 20 or 30 pounds and then put it back on. But that wasn't the case. Not by a long shot.

Now, a little more than 10 months later, Wornick has lost close to 200 pounds. He weighs just 168 pounds and is still adjusting to his new life as a slender man. According to a buzzy blog from Syracuse.com, Wornick still can't help but examine booths in restaurants to see if he'll be able to fit in them, even though he now can easily shift into the open space with room to spare. He's also surprised when somebody accidentally jostles him in a crowd--a far more routine occurrence when he was carrying all that extra weight on his frame.

So how did he shed so much weight? It all started with a bet in the form of a New Years resolution. Wornick's wife, Angelia, challenged him to see who could lose the highest percentage of body weight. The loser would have the winner's name tattooed on his or her stomach. Those big stakes inspired Jay to cut back on eating big steaks. He ate less meat, switched to water instead of soda, and began working out.

At the end of the first month, he'd lost 40 pounds. Inspired, he kept at it, and the weight kept coming off. It's worth noting that Jay was able to do this without a personal trainer, though he did have a lot of help from his local YMCA, which he called "a huge support system." He also consulted the Web for diet tips and found quite a few from Dr. Oz. Jay will make an appearance on the doctor's show in next few weeks.

To make good on her side of the wager, Angela plans to have Jay's name tattooed on her back sometime in the coming weeks. Jay also plans to get a tattoo--now that he's so much slenderer, his wedding ring doesn't fit his ring finger--so he plans to replace it with a ring-shaped tattoo.

New cause of osteoarthritis discovered

New cause of osteoarthritis discovered - Osteoarthritis is driven by low-grade inflammation, which is at odds with the prevailing view it is brought on by joint wear and tear, U.S. researchers say.

Dr. William Robinson of Stanford University School of Medicine said the finding offers hope that by targeting the inflammatory processes that occur early on in the development of osteoarthritis -- well before it progresses to the point where symptoms appear -- the condition might someday be preventable.

"It's a paradigm change," Robinson, the senior author of the study, said in a statement. "People in the field predominantly view osteoarthritis as a matter of simple wear and tear, like tires gradually wearing out on a car."

It also is commonly associated with blowouts, such as a tear in the meniscus -- a cartilage-rich, crescent-shaped pad that serves as a shock-absorber in joints -- or some other traumatic damage to a joint, Robinson said.

It has long been known that osteoarthritic joint tissues host a heightened number of migratory inflammatory cells and of some of the substances these cells secrete -- "not nearly as much as in the case of rheumatoid arthritis, which is clearly an autoimmune disease, but enough to make us wonder if inflammation is also a major player in osteoarthritis as well," Robinson said.

Robinson's team's said the increased numbers of certain specialized inflammatory proteins early in the progress of osteoarthritis, before it becomes symptomatic, suggested that inflammation might be a driver, rather than a secondary consequence, of the disease.

The finding was published in the journal Nature Medicine.

Arthritis treatment identified

Arthritis treatment identified - A new study has identified patients with juvenile idiopathic arthritis (JIA) who are more likely to respond to etanercept treatment. It found that patients who had with low measures of disability at study entry, younger age at the onset of JIA, and fewer disease-modifying antirheumatic, drugs used before initiating etanercept achieved an excellent response to the treatment.

The U.S. Food and Drug Administration and the European Medicines Agency for the treatment of JIA approved Etanercept a decade ago.

"The ability to identify patients who are more likely to respond to etanercept treatment would be an important step toward tailored patient-specific treatment and subsequently could improve current treatment approaches," according to the researchers.

Marieke H. Otten, M.D., M.Sc., of the Erasmus MC Sophia Children's Hospital, Rotterdam, The Netherlands, and colleagues conducted a study to evaluate JIA disease activity after beginning treatment with etanercept and to identify characteristics associated with treatment response.

The researchers used data from the Arthritis and Bioiogicals in Children Register, an ongoing prospective observational study since 1999, which includes all Dutch JIA patients who received biologic agents.

For this study, response to treatment was evaluated 15 months after initiation of etanercept.

Of the 262 patients, 85 were considered excellent responders after 15 months of treatment; a total of 85 patients were considered poor responders; and the remaining 92 patients were considered intermediate responders.

Compared with achieving an intermediate or poor response, achievement of an excellent response was associated with lower scores on measures of disability at study entry; low number of DMARDs (including methotrexate) used before introduction of etanercept; and younger age at onset.

Compared with achieving an intermediate or excellent response, achievement of a poor response was associated with systemic JIA and female sex.

During etanercept treatment, patients experienced a total of 31 serious, 99 infectious, and 179 noninfectious adverse events.
